Providing safe and person-centred support for young people aged 16-17.
Oak Tree Meadows provides supported accommodation for 16- and 17-year-olds who need a safe, stable home environment as they prepare for adult life. Although our homes are based in the West Midlands, Derbyshire, Nottinghamshire and Staffordshire, we support young people from across the UK.
Our service bridges the gap between children’s care and independent living, empowering young people with the confidence and skills to thrive.
Comfortable, safe homes located in local communities that offer a warm, family-style environment where young people feel respected and supported.
Each young person is given an individual support plan shaped around their unique strengths, needs and aspirations, with regular progress reviews.
Hands-on training in essential life skills such as budgeting, cooking, household management and organising daily routines.
A robust safeguarding framework supported by regular one-to-one check-ins and dedicated emotional support through key worker sessions.
Helping young people access education, training and meaningful work opportunities through strong links with local colleges and employers.
Round-the-clock staff support, including sleep-in or waking night staff according to each young person’s individual needs.
Young people work closely with staff to identify realistic goals and build clear, personalised plans for their future.
Encouraging young people to discover and develop their hobbies, talents and creative interests.
Strong partnerships with specialist services such as mental health support, recovery programmes and local community networks.
Our supported accommodation is built on genuine care and stability. We go beyond simply providing housing — we become a trusted partner in every young person’s journey, giving them a solid foundation to grow and succeed.
We believe every young person deserves the opportunity to build their own future — and we’re fully committed to making that a reality.
